Davis AC. The prevalence of hearing impairment and reported hearing disability among adults in Great Britain. Int J Epidemiol 1989;18(4):911–917. doi: 10.1093/ije/18.4.911 The evidence demonstrated the benefit of using earwax softeners before ear irrigation, however the evidence was not strong enough to recommend a specific type of softener between oil, sodium bicarbonate or water. The committee recognised that it is currently common practice to use sodium bicarbonate or olive oil, although there is wide variation in practice amongst clinicians and personal preference plays a strong role, as does the type of the earwax. The evidence indicated that water is as clinically effective as other types of softeners and could be used as an alternative. The committee considered that clinical judgement should be used to determine which softener is most appropriate for the patient. Clinicians should also consider cost as a factor in their decision-making. Swimmer’s ear is an infection of your outer ear. Bacteria usually cause it, but fungi (more than one fungus) can cause it as well. People with swimmer’s ear may also develop pain, itching and swelling. They may suggest using eardrops for a bit longer, or they may carry out a minor procedure called ear irrigation (previously syringing) to clean out your ear canal. Glue ear may be secondary to an acute otitis media (acute middle-ear infection) or a result of eustachian tube dysfunction. It has an incidence of 40% in children aged two years, reducing to 1.4% in those aged 11 years [5] The committee agreed that the critical outcomes for decision-making were successful earwax removal; improvement in hearing; adverse events related to earwax including perforation, infection, vertigo, bleeding and discomfort; hearing-specific health-related quality of life as reported by validated scales including WHO DAS II, HUI2/HUI3, Cambridge Otology QoL Questionnaire and the SSQ. Further critical outcomes included patient reported disability or benefit; measures validated to demonstrate changes with audiology care in the population under study for example, the Device Orientated Subjective Outcome Scale, Glasgow Hearing Aid Benefit Profile and Hearing Handicap Inventory for the Elderly.

The committee noted the clinical benefit of ear irrigation as opposed to ear drops alone. In current practice, wax softeners are the standard intervention in primary care, followed by irrigation if unsuccessful. eardrops – dropsused several times a day for a few days to soften the earwax so that it falls out by itself One UK health economic evaluation was identified, which was carried out as part of an NIHR health technology assessment. This compared 3 options: no treatment; treatment using softeners for a week and self-irrigating (using a bulb irrigator) if the wax does not clear; using softeners for a week and returning to a GP if the wax does not clear. The study described its results as “exploratory and should not be used as a basis for changing policy and practice”. Ahmed OH, Gallant SC, Ruiz R et al. Ear drops are generally safe when used properly. But you should never use ear drops if you have a ruptured eardrum. If your eardrum has a hole in it, the medication can get into your middle or inner ear and cause complications. Can a bacterial ear infection go away without antibiotics? Patients with a recurrent build-up of wax may benefit from the use of water or oil-based ear drops, initially for three to five days to clear the wax, or once per week prophylactically [9] A common ear problem that patients of all ages may present with is hearing loss or deafness. Young children commonly present with fluctuating hearing loss, known as ‘glue ear’, which is caused when the middle ear — normally air-filled — becomes filled with fluid. This fluid tends to look like glue, hence the name. It is important to be aware that some antibiotics — including the aminoglycosides (gentamicin and neomycin), ciprofloxacin, clarithromycin — and chemotherapy agents — such as methotrexate and cisplatin — are known to cause or worsen tinnitus. Medicines used to treat hypertension (such as angiotensin-converting-enzyme inhibitors), hypercholesterolaemia (such as atorvastatin), anxiolytics and antidepressants should also be reviewed as they can potentially cause tinnitus. However, anecdotally, many ENT specialists find the use of sodium bicarbonate ear drops to be the most effective, perhaps as ear wax is water-soluble and the oil-based preparations have only a softening effect in in vitro studies. It may be described as ringing, buzzing, whooshing, humming, hissing, throbbing, music or singing, and patients may hear these sounds in one or both ears, or in their head [14]

The evidence found was very limited with mainly single studies made up of small numbers comparing interventions. The majority of the evidence ranged from low to very low quality. This was mainly due to risk of bias in patient selection and allocation where adequate descriptions were not provided and to indirectness as several papers do not include enough detail to determine whether children were included. A build-up of earwax is a common problem that can often be treated using eardrops bought from a pharmacy. The best ear wax removal treatments you can buy in 2023 1.
